He comes off as motherly at first because he worries about his friends and even the customer who's care he is in. He wants to ensure those around him are comfortable with him as he takes care of others.
Though he comes off at motherly, he tends to get too comfortable and starts pulling off jokes that might come off mean. His older age has nothing to do with how he really acts so sometimes he come off as childish.
